This comprehensive study investigates the transformative potential of patient-centered care (PCC) approaches in chronic disease management through a novel computational framework that integrates artificial intelligence with human-centered design principles. Unlike traditional healthcare studies that focus primarily on clinical outcomes, our research introduces a multidimensional assessment model that captures patient engagement, self-efficacy, and quality-of-life metrics alongside conventional health indicators. We developed and implemented a hybrid methodology combining natural language processing of patient narratives, machine learning analysis of engagement patterns, and network analysis of care coordination across 1,247 patients with multiple chronic conditions over an 18-month period.
